CMFT Community Medicines Optimisation Services
Band 6/7

We are looking for motivated and enthusiastic Clinical Pharmacists to join our Community Medicines Optimisation Team at Central Manchester Foundation Trust.

The Community Medicines Optimisation Team is a small, friendly team located in the community setting. We are an award winning team at the forefront of the local integration agenda, developing innovative new projects and ensuring our patients are supported to get the very best from their medicines. Although community based, the team are an integral part of the Pharmacy Department of Central Manchester Foundation Trust (CMFT). As such we benefit from working for a large, forward-thinking department, including access to training programmes and excellent career-development opportunities.

As a bank pharmacist you will be able to improve patient care by working alongside a wide variety of healthcare professionals in different community settings, including GP practices, Out of Hours Providers, North West Ambulance Service, care homes and Intermediate Care units. The role will also involve supporting our programme of training and governance work in community.

As a bank pharmacist your role may involve:
• working with GP practices to ensure best practice with medicines
• providing clinical pharmacist support to North West Ambulance Service
• providing clinical support to our care homes and Intermediate Care units
• providing medicines information and support to community services staff
• delivering support to local GP out of hours providers
• supporting delivery of our medicines management staff training programme
• supporting a range of service developments and projects in response to the greater integration agenda

You must have a significant amount of hospital or primary care experience. Excellent, up-to-the-minute clinical knowledge, attention to detail and good time
management are essential and you must be a registered pharmacist with the General Pharmaceutical Council (GPhC). You will need to demonstrate good written and verbal communication skills, have good organisational skills and an ability to work independently and as part of a multidisciplinary team. You will also need to be able to drive and have access to a car for work purposes.

We have short and long term bank work contracts available. Flexible working hours can be agreed.
