Community Care Navigator

The Manchester Local Care Organisation (MLCO) is looking at developing this exciting role in Partnership with Trafford CCG. The Community Care Navigator Service currently services the people of South Manchester and has now been expanded to serve the people of Trafford as well.

The post holder will act as a community liaison and coordinator based in both Wythenshawe Hospital and South Manchester/Trafford community settings to work alongside teams to improve communication and collaboration including signposting and case managing citizens to ensure they have access to the right support, at the right time, in the right location. The service has a large community based focus and links directly with people within their own homes and collaborate with staff members in community teams.

The post will work alongside members of the multi-disciplinary team to enhance communication and connection leading to leading to more effective citizen care with a focus on keep people safe, well and independent in the community. The post holder will also be able to manage their own diary and caseload autonomously.

The successful candidate will have a key role in navigating health, social care and 3rd sector systems. They will act as a vital link to ensure care is effectively managed outside of the hospital setting where appropriate reducing the risk of unnecessary hospital admission.

The successful candidate will require confidence and diversity with a range of computer skills. You will also have good communication and demonstrate exceptional customer service skills alongside problem solving skills and the ability to prioritise workload.

The post is band 4 (£20,150 - £23,363) and is 37.5 hours over a 7 day rota, based within Wythenshawe Hospital Integrated Discharge Team and South Manchester/Trafford community settings.
