We are looking for a Band 6 Nurse to join our  dynamic team providing Crisis Response Service in South Manchester. The Crisis team is part of the integrated Manchester Community Response Service which includes Discharge to Assess and Intermediate Care & reablement.   

As a Band 6 Nurse, this is an exciting time to be part of our developing and progressive working environment which will deal with ‘crisis’ referrals from community, primary care, A&E colleagues & NWAS, and where your clinical skillset will contribute to local residents remaining in their own homes and preventing unnecessary A&E attendances.

Our integrated (health & social care) Crisis Response team is made up of Advanced Practitioners, occupational therapists, physiotherapists, nurses, support workers, pharmacists and social care staff. The Service provides 7 day, extended hours (08:30 - 22:00)  working providing  urgent and timely response to referrals. From this, a rapid assessment and community intervention for up to 72 hours will support adults with acute / crisis health or social care needs, at home.

As well as bringing your clinical skills to the team, you will be a champion of integrated community care. You will be a strong team player, an excellent communicator, adaptable, committed, well organised and work well under pressure. You must be able to travel around various community locations delivering care.  You must be motivated, enthusiastic and have proven experience of working in a multi-disciplinary team.
