We are looking for registered Learning Disability Nurses to join our Multi professional Community Adult Service to work within our Positive Additional Support Services which includes the Complex Needs & Short Breaks Service and our City Wide Community Learning Disability Teams.

You will be working as part of a nursing led team with wider support from the multi-disciplinary community based team. The role will involve leading and contributing to the effective provision of services to people with a learning disability, complex needs and Autism. You will liaise with a wide range of both statutory and non-statutory agencies to provide creative support within the day time and short breaks service. The role is focused on providing excellent health outcomes for Adults with Learning Disabilities. You will be assigned to the Complex Needs Day Service, Short Breaks Service or a City Wide Community Learning Disability Team on a rotational basis.

You will be involved and work directly with individuals that use the service, their families and the people who support them in the assessment and planning processes. This will require you to have a broad range of communication skills and be able to demonstrate an evidence based approach using a model of Positive Behavioural Support.  

The service is proactive in the development of both staff & service and is committed to CPD providing regular specialist multidisciplinary support & education with regular reviews & supervision.
