This is an exciting time to join our Discharge to Assess team in south Manchester supporting patients in their own homes after discharge from hospital. 

This multi professional, integrated health & social care team is one of the teams in that make up the Manchester Community Response Service in south Manchester.

The team is now looking to recruit an innovative, enthusiastic Band 3 Therapy assistant / support worker to join therapists, nurses, social care staff and other support workers working in the community.

Working as an integral part of the multi-disciplinary team you will help support the patients and their families & carers after discharge for up to 7 days. 

Staff  work over 7 days a week and service hours are 08:00 - 20:00

The job also involves independent travel within the local geographical area to deliver the role.

With the above in mind, you would ideally have experience working  in the NHS and possibly some experience in community settings.
