Discharge to Assess in Central Manchester is expanding - if you like a challenge then this is the job for you!

This is an exciting time to join our expanding team in central Manchester as we further develop the Discharge to Assess element of the service. Now part of the Manchester Local Care Organisation, our service is looking to recruit an innovative, enthusiastic Band 6 Occupational Therapist to be part of a lively and dynamic multi-professional Community team comprising of health and social care staff. 

Working as an integral part of the multi-disciplinary team you will facilitate timely discharge from an acute hospital or community beds and provide specialist occupational therapy assessment, treatment and pathways of care. Being part of the Discharge to Assess process you will liaise with colleagues across all sectors to arrange appropriate support in the community and will be managing a caseload of patients ensuring a high standard of Occupational Therapy provision. 

Discharge to assess will be a seven day service, therefore this entails working weekends, bank holidays and late shifts on a rota basis. The Post also involves working within the main hospital site and delivering assessments & interventions to patients in their own homes/ place of residence. Independent travel within the local geographical area to deliver the role is essential.

The service has close links with the Intermediate Care service. As the wider Manchester Response teams develop staff may be rotated through this service.

With the above in mind, you must have experience of working in the NHS and it would be desirable that you have experience of working in a rehabilitation/ community setting. You are able to demonstrate an appropriate level of clinical skills in occupational therapy, you are experienced in person-centred assessments, as well as developing, implementing and evaluating programmes of care. You must have a recognised Occupational Therapy qualification and be HPC Registered.
