We are Manchester Local Care Organisation - an exciting, new public sector organisation that is bringing together NHS community health services and social care services in the city. We’re driven by the aim of improving the health of local people in Manchester by working across organisational boundaries.

The first health and social care staff have been deployed to us from April 2018 and are now starting to work in new ways across the city. Our teams work as one in the neighbourhoods they serve – delivering and designing services in partnership with local people. We also provide key services that work across wider localities and city-wide. We are looking for Band 6 Nurses to join our dynamic team which will be rolling out Crisis Response across Central and South Manchester. Building upon the national success of the North Manchester Crisis Response Service, Manchester Local Care Organisation (MLCO) is now backing the expansion of Crisis Response across the whole of Manchester.

As a Band 6 Nurse, this is an exciting time to be part of our developing and progressive working environment which will deal with ‘crisis’ on a daily basis, and where your clinical skillset will contribute to local residents remaining in their own homes and preventing unnecessary A&E attendances.

Our Crisis Response Service will operate as an Advanced Practitioner clinically led multi-disciplinary team comprising of Advanced Practitioners, occupational therapists, physiotherapists, nurses, support workers, pharmacists and social care staff. The Service will provide a 7 day, extended hours, urgent and timely response to referrals from the ambulance service, acute or primary care and other health or social care professionals. From this a rapid assessment and community intervention for up to 72 hours will support adults ‘in crisis’ with health or social care needs, at home.

MLCO has an ambitious vision for integrated health & social care services across Manchester, including the new Crisis Response Service. A number clinical posts are now available in both the central and south areas of Manchester. Applications are welcomed from qualified and experienced staff.

As well as bringing your clinical skills to the team, you will be a champion of integrated community care. You will be a strong team player, an excellent communicator, adaptable, committed, well organised and work well under pressure. You must be able to travel around various community locations delivering care.  You must be motivated, enthusiastic and have proven experience of working in a multi-disciplinary team.
