Central Manchester University Hospitals NHS Foundation Trust (CMFT) and the University Hospital of South Manchester NHS Foundation Trust (UHSM) will join together to create a new Foundation Trust in October 2017 which will be named Manchester University NHS Foundation Trust; subject to required approvals.   Following this, the plan is for North Manchester General Hospital (which is currently part of The Pennine Acute Hospitals NHS Trust) to join the new organisation around 12-18 months later.

As a result of this plan, it’s such an extremely exciting and challenging period in the delivery of health and social care in Greater Manchester and one that provides a wealth of opportunity. Our new organisation will provide a single hospital service across the city of Manchester and in 12- 18 months will also include the services currently provided by North Manchester General Hospital.  These changes will make a real difference to the health outcomes and quality of life for the people of Manchester. 

This is an exciting opportunity for an occupational therapist with experience in stroke to build and enhance their skills in the early supported discharge team based at Trafford General Hospital.

We are looking for an enthusiastic, forward thinking and adaptable individual with a commitment to providing specialist evidenced based assessment and treatments to stroke patients and leading the multidisciplinary early supported discharge team. You will provide clinical leadership within the multidisciplinary team, delivering high quality rehabilitation and support to service users in the early weeks immediately following discharge home. There is currently an expectation to provide some cross cover with the inpatient stroke service at TGH.

The specialist clinical knowledge and application of neurorehabilitation skills to deliver high quality intervention within the patients’ homes. The ESD clinicians and assistant work as an integrated team to provide individually tailored rehabilitation programmes to address patient identified goals. Flexibility, and good organisational and prioritisation skills are essential to this community-based role.

You will provide day to day clinical team and operational leadership for the ESD team, and will be supported by a Clinical Lead therapist
